Reformative Training Centre (RTC) Case Management
Supporting Youths Through Rehabilitation and Reintegration
In April 2024, YGOS and REACH Community Services formed a consortium and were appointed by the Singapore Prison Service (SPS) to provide case management services for youths undergoing Reformative Training.
Together, we support youths through their rehabilitation and eventual transition back into the community, helping them prepare for family life, education, employment and the responsibilities that come with moving forward.

YGOS AT RTC
Individual Support Throughout the Journey
Every young person enters Reformative Training with different experiences, challenges and circumstances.
YGOS works individually with youths to understand their needs and provide support throughout their rehabilitation. Through regular engagement and case management, youth workers help them reflect on their experiences, work towards personal goals and prepare for what comes next.

Preparing For Reintegration
Returning to Everyday Life
Leaving a structured environment and returning to the community can require significant adjustment.
YGOS works with youths to prepare for this transition, including the practical and personal challenges they may face when returning to their families, education, employment and community.
Where appropriate, this also involves working with family members and other agencies supporting the young person.

Aftercare
Continuing the Relationship
Support can continue after a young person leaves Reformative Training.
Through YGOS's aftercare work, youths can stay connected to people who know their journey and continue receiving guidance as they adjust to life back in the community.

Working With Partners
Supporting Reintegration Together
Successful reintegration involves the people and systems around a young person.
YGOS works alongside families, agencies and community partners to strengthen the support available to youths as they move forward after Reformative Training.
